The Club is undertaking certain investigations with respect to members’ golf equipment and belongings that were destroyed in the fire.
As part of those investigations, the Club requests that any members whose equipment and or belongings were destroyed in the fire to complete the form that follows. All fields must be completed and please provide full and clear information.
